Types of Transportation Available to Campus
Bradley International Airport in Windsor Locks, CT is about 30 minutes from campus. Logan International Airport is located in Boston, MA approximately 2 hours from campus. Amtrak trains and Greyhound and Peter Pan Bus Lines operate out of Downtown Springfield. The campus is located on a public bus route providing service to and from the bus station and train station. Taxis are also available to provide transportation to and from campus.
Driving Instructions to Campus
From the East and West (Boston, Albany) via the Massachussetts Turnpike (I-90): Leave the Mass. Pike at Exit 6. Turn left onto I-291. Take Exit 5 off I-291 ("Route 20-A West to East Springfield"). Bear right at the end of the exit ramp on Page Blvd. Take the left at the first light onto Roosevelt Ave. Take Roosevelt Avenue 2.5 miles to the intersection with Wilbraham Road (fifth traffic light). Turn left onto Wilbraham Road and follow it 1.5 miles to the third light. Turn right into the parking lot of the Welcome Center. (Total 5.6 miles from the Mass Pike.)
From the North via Interstate 91: Leave I-91 at Exit 8, ("Ludlow, Boston I-291"). Travel to Exit 5B, ("East Springfield"). Turn right off of the ramp onto Page Blvd. At the first traffic light, turn left onto Roosevelt Ave. Take Roosevelt Avenue 2.5 miles to the intersection with Wilbraham Road (fifth traffic light). Turn left onto to Wilbraham Road and follow it 1.5 miles to the third light. Turn right into the parking lot of the Welcome Center. (Total 8.6 miles from I-91.)
From the South via Interstate 91: Leave I-91 at Exit 2, ("East Longmeadow"). Take the exit ramp following the signs ("Route 83") to the light at the intersection of Longhill and Sumner Ave. Turn right onto Sumner Ave. Travel straight on Sumner Ave. (which becomes Allen St.) to the traffic light at the intersection of Allen St. and Bradley Road (3.2 miles). Turn left onto Bradley Road and travel 1.6 miles to Wilbraham Rd. and turn right. Travel 0.2 miles to the next light and turn right into the parking lot of the Welcome Center. (Total 5.7 miles from I-91.)

In general, documentation should include the following: ? a clear statement of the disability including a specific diagnosis and prognosis; ? a description of current functional limitations relevant to the diagnosis; ? a summary of evaluation procedures including diagnostic tests, scores, and evaluation results; ? a description of the implications that the functional limitations may have on major life activities within a university setting (e.g. academic, residential life, etc.); ? a summary of relevant historical information (e.g. educational, developmental, medical, etc.) and; ? recommendations for reasonable accommodations and a rationale explaining how specific accommodations address the functional limitations. Information provided about a disability must be on letterhead signed by a licensed clinician who is qualified to make the diagnosis. IEP'S 504 plans Medical documentation The date the letter was written, the provider?s written name, the provider?s state licensure/certification number, professional title, mailing address, telephone number, and provider?s signature. A diagnostic statement identifying the disability/diagnosis (with the date of diagnosis/onset). A summary of the relevant education, developmental, psychiatric, medical history that supports the diagnosis Severity of the disorder/diagnosis (mild/moderate/severe). Medication and/or treatment currently prescribed. An assessment of major life activities that are affected by the impairment/diagnosis (examples: concentration, memory, social interactions, class attendance, learning, reading, thinking, etc.) Clear and specific evidence and identification of the disability diagnosis, using direct language and avoiding terms such as, ?suggests? or ?is indicative of? or ?is consistent with.
